O U R M I S S I O N
Hands at the Cross is a residential home for adults in the beautiful suburban setting of Stillwater, Minnesota. We provide services, care, and support to those whose needs are changing but want to maintain the familiarity and comfort of a true "home."
OUR MISSION: To honor God by ministering to the lives of older adults by providing an individualized approach in the delivery of care and services.
OUR VISION: To reflect the love of God as we partner with family, caregivers, and community to determine the care plan for you and your loved ones as an expression of God's grace.
OUR COMMITMENT: To serve with love, compassion, and respect, aiming to make our residents comfortable and happy in our home. We desire to live under God's grace; to love and serve others with the same grace.

Hands at the Cross Care Home is open to serve adults aged 18 yrs and older with general age related functional impairments, physical and developmental disabilities, brain and spinal cord injury, heart disease, post-op recovery needs and age related diagnosis such as Parkinson's. We also service adults with tracheostomy and patients who are ventilator dependent as well as stable substance abuse diagnosis.

Here at Hands at the Cross Care Home we strive to create a balanced living experience for all of our residents; the care we provide is coordinated with your loved ones specific health concerns and lifestyle interests in mind and we treat each individual with respect.

We believe that your questions and concerns regarding the care your loved one receives is critical to the individually tailored services we provide. We understand making long term care decisions is difficult and we don't take those decisions lightly; it is our goal to make your family member's transition into one of our homes as seamless and smooth as possible.
Prior to accepting your loved one into one of our homes, we will conduct a detailed assessment to identify their needs and make clear their present medical situation. We evaluate specific concerns in consultation with the perspective resident, his or her family members and the appointed healthcare providers.
Our Care Homes are owned and operated by a registered nurse with 30 years of direct patient care experience. She involves herself intently in the day to day care of her residents and the training and supervision of her staff. Her goal is to provide a caring and educated staff that strives to engage each resident by managing their total well being while attending to their personal choices, independence and dignity.
We engage our residents on a daily basis and we coordinate activities tailored for them to realize their individual goals as set forth in their care plan. With these activities we strive to encourage socializing with other residents and staff and promote general well-being. More specifically, we manage your loved ones medical situation, doctors appointments, medications and daily ADL requirements with detail and accuracy. It is our goal to see that each individual's needs are addressed as if they were being cared for by family.

245 D SERVICES
RESPITE
A short-term care service intended to provide a break to an individual's primary caregiver(s). Respite may be provided in the Respite provider's home or the individual client's home.
HOMEMAKER SERVICES
A service that provides light household cleaning and laundry to assistance with home management and/or activities of daily living (ADL's). Home management services include meal prep, shopping for food, clothes and supplies, simple household repairs and arranging for transportation. ADL's include bathing, toileting, grooming, eating, dressing and assistance walking.
ADULT COMPANION SERVICES
A non-medical care, supervision and socialization program provided to an individual over the age of 18. Adult Companion Services help an individual to gain more independence and accessing their community.
NIGHT SUPERVISION SERVICES
A program of overnight assistance and monitoring provided by an awake staff in the individual’s own home.
PERSONAL SUPPORTS
A non-medical assistance and supervision care program provided in the individual’s home or community. This one-on-one service is intended to increase one’s independence and involvement with community activities.
* Above services are available under a variety of payment options including, Medical Assistance Home and Community Based Waiver programs (EW, CAC, DD, CADI) as well as private pay.
LICENSE
Hands at the Cross Care Home (HATCCH) is an assisted living facility licensed to provide services under Minnesota Department of Health's Comprehensive Home Care License, subject to the standards of Minnesota Statutes, 144A. HATCCH is also licensed under the Department of Human Services to provide home and community based services to persons age 18 + older with disabilities. HATCCH is subject to the standards of Minnesota Statutes, Chapter 245A & 245D.
